       – Complete an application. 

       – A “MY STORY” essay of 500 words or more expressing current goals &  objectives & other personal information 

       – Official High school or College Transcripts 

       – Photocopy of SAT or ACT results (optional) 

       – Letter of acceptance from a two or four-year College or University 

       – A copy of the death certificate 

       – Copy of the military form DD-214 (if applicable). 

       – One or two letters of reference from a current or former teacher, coach, employer, minister or anyone else that can attest to   the applicant’s character 

       – A document showing your relationship to the deceased
